Note:
It's debateable whether sometimes he uses an f#minor instead of the D/F#. It's
really hard to hear it. I chose the D/F# because to me I hear the F# bass note
but not necessarily the full minor sonority of the F# minor chord. That's my take
on it. To play D/F#, you just wrap your thumb around the neck and grab the F# on
the 6th string 2nd fret while you're playing the D chord. It's very useful for
walk up bass patterns.
Also, the transition at the end of chorus two is just the intro stuck in there.
